Dr. Sarma is one of the few USCIS Certified civil surgeons in the United States. She is authorized to preform Physical Exams by an appointment with flexible hours. Please call the office to schedule.
Requirements
1. Applicants are advised to fill out the I-693 Form available on the USCIS Website. One form per applicant.
2. Bring immunization records.
3. Arrive 15 minutes before your appointment to complete the paperwork.
